To update details like your name, address, phone numbers or email address:
- fill out a change of contact details form
- phone or visit the surgery from 8am to 6pm
You may be required to produce identification of any changes.
If you change your address to somewhere outside our catchment area, you may be asked to move to a GP Practice closer to home. This is to ensure you receive access to appropriate medical services in your area.
